Susan Taki awakened from a magic-induced coma in the events of The Death Brothers: A Supernatural Awakening. After helping her boyfriend, the demon hunter, Father Tom Padilla in New York, she has now returned home to San Diego. She is weighing an offer from the vampire slayer, Jericho Caine, to be his partner, while she patrols the streets as a supernatural policeman at night.

The ongoing case of "The Cloud Conspiracy" is taken from the files of special agent Tray Marrow. A super-intelligent UFO crash survivor is held at a quasi-government research company named Genesis 15. He is given the name Lawrence. As an advanced species, he creates an implantable microchip enabling humans to become one with the cloud. A power-hungry loner named Larry Fledges realizes the power that the microchip holds. He steals the chip and self-implants becoming "one with the cloud". The clock is now ticking for special agent Marrow to locate Fledgers before world chaos ensues.

Two weeks have passed since the events of the Hangman Returns. Jericho Caine and Father Tom Padilla, the Death Brothers, return to The Reefs in the Gulf of Mexico to settle a score with the vampire sirens. However, a female angel, Sariel, with romantic intentions toward Jericho, plans to separate the brothers and seduce the slayer. Sariel strikes a deal with the Hangman to distract the brothers by casting a spell that resurrects vanquished supernatural beings in New York. In return, Sariel agrees to help the Hangmen bring back his wife and son, who are in heaven.

The Hangman has returned from the unknown dimension he was sent to at the end of The Death Brothers: A Supernatural Awakening. Now, he only has vengeance on his mind for the people who he once considered friends and allies. Leechadon the Soul Eater, a powerful demon and the new leader of the underworld, strikes a deal with Hangman to help him get his revenge, in exchange for the Hangman agreeing not to interfere with Leechadon’s plans for the Earth.
